If you have received a letter from CHI about the HSE Cyber-Attack, you can request copies of your breached documents.
You can request your breached documents by post. This is the only way to request these documents. You can no longer do this online.
What you need to do

How long your documents will take?
It can take up to 1 month for the documents to be ready from the date that we receive your completed form and photocopy of your ID and / or evidence of legal guardianship.
In some cases, it may take up to 2 months longer. We will let you know if this happens and explain why.
When we send you your documents we will include details of how to contact us. This is if you want to speak to us about the information you receive.

Online Data Subject Access Requests submitted before 26th May 2023
If you previously requested your documents through the online portal before 26th May 2023, you will still be able to view these online.
We will send you a text message when they are ready to view or download. It can take up to 1 month for the documents to be ready but we will try to get them to you as soon as we can.
Log in to your account to view and download documents related to you when they are ready.
If you registered but did not make an online Data Subject Access Request before 26th May 2023, you can now only make a request by post.
